[TYPO3-core] RFC: Accessible radio buttons

This is a CVS patch request.

Type: Bugfix

Description:
When activating the accessibility mode in forms, the radio buttons have all
the same id.
Additionally, they are not wrapped with label tags. Instead, the field
description is wrapped.

Solution:
The patch fixes this by adding _<nr> to the IDs and wraps all radio elements
with appropriate label tags.

Additionally, the trailing <br /> after each line has been removed, it is
now set by the TS templates, so it can be configured.
