When a user's record is deleted from the FE or in the BE, does this mean 
that
(1) the image file shall be deleted completely?
(2) Or shall only the record be marked as deleted?

If the files were deleted, this would save disk space. However it would 
not allow to enable the record again, because the images would be lost.

I would vote to delete the files or move them to a deletion folder from 
where they could be backuped and deleted from the filesystem.
