The problem was that I had your session_start() fix applyed onto 4.5.6 
for #29274 [1] with the patch attached to the forge issue.

You later seemed to have enhanced that patch in commit 3863b1be [2], 
which will be included in 4.5.7, and with this, the problem seems to be 
gone.
