No sooner written than I sorted it out.  It was a permissions problem.

When installing extensions folders were being created with permissions of:  drwxr-sr-x

If I changed them to drwxr-xr-x the site worked.

I changed the foldercreatemask setting in the install tool to write as 0755, instead of 2775 and now we seem to be good.
