[TYPO3-templavoila] Typo3 4.2 docheaders for templavoila

Hi

I have in tm_tvpagemodule Typo3 4.2 style layout for templavoila, when 
used with Typo3 4.2.x (for older versions more conventional layout).

Best layout using skin_grey_2 but should work also with t3skin.
