[TYPO3-Solr] "Templating: Facets with Checkboxes" example ?

Hello,

I thought my typo3/solr project was complete, of course it wasn't: some users want to have checkboxes next to facets... and I have to say I agree with them

So I found the page

but there is no form tag, neither submit button. I tried to include some markers but only got headache

would anyone be nice enough to give me a complete example?
