The guys who did philips already offered to make a case study. Johns Kopkins
would be great, as the edu.typo3.com website is in the making anyway and we
have too few cases outside of Germany and Austria.

Yes, you need their permission, but that should be not too hard to get.
Sander once made a form, but i am not sure to what end. (sorry)
