Yes, for each release you will find upgrade instractions as links below the 
release notes:
http://typo3.org/download/release-notes/typo3-4-3-release-notes/

In short, upgrade in small steps, e.g. 4.1->4.2->4.3->4.4
Run the update wizard after each step and update extensions and compare the 
database
check if your site still works and check the deprecation log

Create a dump and play this in a test environment before doing the upgrade 
on a live site.

There are some changes to DB, but the content should be preserved.
