[TYPO3-english] FE upload with title input

Hi,

I'm using customized upload solution from 
https://github.com/helhum/upload_example to have file upload from a FE 
extbase form into FAL. However, I'd need to extend this with input where 
users would give title for uploaded image.

Unfortunately, I have no idea where to start with that. Which part needs 
to be extended? ViewHelper, converter, something else?
