The Upgrade Wizards in the Install Tool do a lot of conversions for you. 
The Install Tool can also check if all the installed extensions can be 
loaded in the backend without causing fatal errors. If an extension is 
found which will cause a fatal error you get the option to deactivate 
the extension(s) to have a working backend.

To prepare yourself for a migration with an extension "LTS to LTS smooth 
migration report" [1]. This is tool that analyses your existing 
installation and reports potential problems
