[TYPO3-english] why is my action getting called twice?

It has become clear that the call to the action is being sent twice - when I type the url in and send it, it is getting redirected to itself thereby hitting the same action twice ( I can see that in chromes network view).  How can that be?  what sort of  config error would cause that?

   I am using typo3 v 4.5.32
