Thank you, the hint regarding the filtered extensions was very helpful. It seems that in 6.2.3, requests from the images / text with images content elements to the file list get "sys_file" as value for the allowed extension list, which breaks everything. I'll try to reproduce this on a fresh installation and if successful file a bug report.
