[TYPO3-english] comments extension with file upload

Hi

I'm looking for a possibility to create discussion pages for registered 
users in TYPO3 4.5 (protected by felogin).

The discussion is one-threaded. No forum. Just comments.
But there should be the possibility to upload files and images for all 
registered users, and decent notifications.

Is there an extension you can recommend?

Or is there a base you can recommend to extend it in this way?
