Hard to say. It really depends on how much deprecated code your extensions use. We recently upgraded from 4.4.5 to 4.5.12 and then 4.7.1 with only minor problems (e.g. GPvar > _GP) and of course outdated Powermail, DAM, Templavoila. Other than that, everything went fine. Just remember to go to the Install Tool and work through the update / compare database scripts.
