It's the most unreliable in the sense that it's hard if not impossible 
to track what's going on, what is going wrong, and if or why sending a 
mail has gone wrong.
On the other hand it's nearly impossible to find a PHP installation 
where mail() is not configured.

To make TYPO3 work in as many installations as possible straight out of 
the box we decided to use the "mail" transport as default.
Every integrator should check the Install Tool options and configure the 
various options in the most optimal way for his/her needs and the server 
situation.
