[TYPO3-english] Simple poll that registeres voter info

Hi

I have been looking at a few extensions for a poll but none seem to 
deliver what I need.

I need a poll where its possible to vote but also send name, address and 
email of the voter.

Also, the result of the vote needs to be displayed in the frontend(have 
been looking at survey modules but these don't seem to support that)

Any good ideas?
