Better to use some of the extensions, because they gives more flexibility.

But running a TYPO3 3.8.1 isn't a good solution at all. Functionality 
and security. At least I hope the installation is patched, 
http://typo3.org/teams/security/security-bulletins/typo3-sa-2009-002/.

If not, it's only luck that the website isn't compromised.

Personally i don't support customer if they don't agree to have a 
contract with me stating that the installation (both core and 
extensions) always should be updated to latest secure version.
