[TYPO3-english] Powermail Extension generating 2 e-mails per submission

I have a number of Powermail Forms that when submitted I will get 2
e-mails kick backed to the e-mail address that is being inputted.  I'm
at a loss of where to start looking as I couldn't find any relevant
information on this behavior anywhere else in the mailing list.

 

So any direction would be greatly appreciated!
