Hi Diederik,

in your list the DB COMPARE is missing. It is VERY important as it 
updates database tables to the structure needed by the new 4.2.3! Go to 
Install Tool -> Database  and click COMPARE, then confirm the first 
block of changes. You do not need the second and third block (renaming 
and deleting outdated db fields).

I recommend this order:

Update Wizard
COMPARE
Clear configuration cache (either by deleting the temp files as you did 
or by using the yellow arrow in the upper right corner of BE)
Update all extensions
Update reference index



 > $TYPO3_CONF_VARS["SYS"]["compat_version"] = '4.1';
 > Because there was also this line:
 > $TYPO3_CONF_VARS['SYS']['compat_version'] = '4.2';

You can do that if you want to clean up - but you don't have to as only 
the last entry found is relevant.
