Ok, maybe second time will be better

replace 'EXT:kopbsurveyext/class.kopbsurveyext.php:tx_kopbsurveyext';

with
'tx_kopbsurveyext';
because you just included kopbsurveyext/class.kopbsurveyext.php with

require_once(t3lib_extMgm::extPath('kopbsurveyext').'class.kopbsurveyext.php');
