Not really my department, but with almost every TYPO3 extension
including download managers, it's a matter of making db records in the
List Module and inserting records on the page to display them.
Presumably in a download extension, you'll also be setting up a
directory with .htaccess or httpd.conf restrictions (or Windows
equivalents)...

Other than that, it'll just be a matter of setting up your FE users and groups.
