[TYPO3] Service Temporarily Unavailable when creating new records

Hello Everyone,
I have a problem in the backend of my installation in that I get the 
Apache error, "Service Temporarily Unavailable", when I try to create 
certain new records. I first encountered this problem when I was 
creating DAM category records, but, today I see that the same error is 
generated when I create a new "Mail form" content element by clicking on 
"Mail form" in the "Form elements" list in the back end.

I tried every other content element in the list and all of them work 
except for the mail form. I can select general plugin to get a generic 
content element then from that form switch to mail form.

When I got the service unavailable error I opened the frame in a new 
window to copy the following URL.

I would greatly appreciate it if someone could give me some ideas on how 
to debug this. I'm running TYPO3 4.1.1.
