[TYPO3] PDF as Image

For some reason, using PDF as an image source is no longer working on my 
installation. I went into the install tool and verified it was able to 
read PDF and display it, but when I create an image content element, no 
luck (where the PDF should display on the website it is blank).

Any ideas of what might have changed or where I might look to get this 
working again? I am running Typo 3 4.0.1.. Thanks!
