[TYPO3] da_newsletter_subscription, user fields not appearing

hi

im setting up the da_newsletter_subscription plugin, and all seems to be 
working nicely, except when i use the module to review subscribers, the 
user defined form fields i created to gather more ino do not appear un 
the module when you choose subscriber list dropdown.

in the tutorial, it shows the user fields appearing on this page.

I went thru the manual several times, but nothing obviously wrong. Any 
suggestions?
