[TYPO3] Install of typo3 4.0.2 stops cold

I'm attempting to install typo3 4.0.2 fresh and having problems. I've
uploaded to a server on which typo3 3.6 and 3.8 are working fine. I've
adjusted the security settings on the files the same as I would for 3.6 or
3.8. Yet, when I run the install script I get the database error then a
blank screen. It's as if the install script is failing to render the html
for itself properly. All I get is an empty page:

Could anyone please advise me as to what I'm forgetting?
