[TYPO3] Extension Manager Problem

Hi list,

PROBLEM: 
When I click the "Ext Manager" link, I get a "Page cannot be displayed"
error, or I just get a completely blank screen.

WHAT I HAVE TRIED:
I have pored through the list to try to find an answer to this issue and
nothing I tried fixed it. Here's what I tried so far:

-- Set the memory limit in php.ini as high as 40MB
-- Set the maximum execution time in php.ini to 120
-- Looked for extensions.bin and extensions.xml.gz and they do not exist
in typo3temp

CONFIGURATION NOTES:
-- MS Windows Server 2003
-- Typo 4.0
-- Apache v.2.0
-- PHP v.4.4.2

Any insights would be greatly appreciated.
