[TYPO3] reminder extension

Hey,

Does anybody know some extension has the following features:
  1,In the Front End there's a form to collect user's basic information 
such as name, birth date etc.
2, This extension will send me an email 2 weeks prior to the user's 
birth date.

I just want to ask before I'm going to deep on the way of coming up a 
new extension by myself.
