[TYPO3] I want to develop extensions - where to start?

Hi

I have been working with Typo3 for 4-5 months and its time to learn how 
to make extensions who can do things TYPO3 and existing extensions cant 
handle.

Where is a good place to start?

I have been playing a little with the kickstarter extension but only to 
add a few fields to an existing table in the database.
