[TYPO3] Direct Mail Subscription + Categories

Hi,

I've created a few categories for my direct mails. When placing the 
extension direct_mail_subscription on a page, I can select categories in 
the backend, so it seems I have the proper TS settings. However I (and 
thus website users) am not able to select any categories when 
subscribing to my direct mails.
Seems like the marker ###CE_CATLIST### is not properly replaced or I 
missed a setting. Any clues how to fix this?

Version info:
Typo3 4.0.2
Direct mail: 2.2.0
Direct mail subscription: 1.1.0
