[TYPO3] Typo3 4.0 Default Login Form failure

Hi Jennifer,

	When you upgraded from 3.8.1 to 4.0, have you done the following??

1. Go to install tool and upgrade to 4.0
2. Go to install tool --> Database --> update all table
