[TYPO3] dmail + tt_address: enter subscribers manually

hello,

I need to subscribe a lot of people manually to a direct_mail newsletter 
(=> tt_address).
I found two ways to do this:
(1) csv-import in dmail plugin
(2) add tt_address record

With (1) I don't know which fields it expects and in which order.
Is it exactly as in DB? I tried exporting tt_address and importing
that, but it doesn't seem to work (only one column per user shown).

In (2) there is no input field for specifying categories
(tt_address.module_sys_dmail_category).
