[TYPO3] how to add alt tag for image?

Do I have to install the 'Alttext for images' (dmc_image_alttext) to 
make alt and title show up in the page source?  dmc_image_alttext shows 
the alt-text when you move the mouse on the image.  But I just want to 
see the alt-text when the image is not availabe.  Any commentes?
