[Typo3] Current menu item on top (first in the list)

Could anyone give me some hints how to use IF to gain this.
Will something like

temp.sth = HMENU
sth.1.if {
  .
  .
  .
}

..work?
I can't figure it out how to mix it with current page id and stuff....
