AFAIK no patent holder has gone after an implementer of any of the mentioned 
techniques in the US. Yes.

Nevertheless, the examples are a clear indication of the completely brain 
dead attitude of the US patents office. For more on patents I would imagine 
that Groklaw [1] has more information than anyone would like to know about.
