[Typo3] Flexible Content: Data Structure ERROR: No Source Value

I am having a difficult time working with Flexible Content Elements in Typo3 
3.8.

I successfully seutp several page templates that work beautifully, however I 
have not been as successful with FCEs. I followed the instructions per the 
Futuristic Templates document (Creating a Basic Flexible Content Element) and 
inside the TemplaViola control center everything looks fine and the DS/TO 
show up under the Flexible CE as expected and the element is stored in the 
same storage folder as my page templates.

On a page, I add a new content element select type "Flexible Content" and when 
the frame reloads, I am presented with the following messages at the top:

Warning: mysql_fetch_assoc(): supplied argument is not a valid MySQL result 
resource in /usr/local/www/typo3_src-3.8.0/t3lib/class.t3lib_db.php on line 
741

and the following in the Content region: 

Data Structure ERROR: No source value in fieldname "tx_templavoila_ds"

Under Page Template Structure I am able to select a Page Template and on 
refresh, the errors go away. However, this pull down does not show my FCE's. 
As a result, if I save FCE's as page templates, I am able to select them and 
use them .. however, this doesn't seem correct (as per the documentation) as 
it renders the "FCE" DS/TO useless.

Any thoughts or suggestions on how to make this work correctly is greatly 
appreciated.
