I cannot support you with patching, but maybe you want to try this 
version of realurl instead, which basically is a fully compatible and 
fully namespaced variant of the original realurl 1.12.8, which I created 
for my projects:

Please consider that realurl class names have changed, so you have to 
adapt the according line in your configuration to:

'userFunc' => 'Tx\\Realurl\\UriGeneratorAndResolver->main'
