It makes hard to create high load projects without thinking about such 
details. You still have to think about indexes and how your quieries 
execute unless you are ready to throw extra 10 servers for each new 
thousand users...

Yes, I've seen such project ;) Not a TYPO3 project, but Java/Hibernate-based.
