we are using our own custom solution available here:
http://typo3.org/extensions/repository/view/neustastaging/current/ in at
least three projects at the moment. It works pretty well but covers only
the editors side, so if you have a new extension you still will have to
install it by hand on the production environment after it was tested on Dev.
