It is because people do not run sites with devIPmask+sqlDebug enabled and do not use many extensions that produce cHash. I can tell that I see cHash warnings several times a week and I do not go to that site often, so it really happens. It is a cache_hash duplicate value warning.

It depends on the site, of course.
