In the new skin for Typo3 4.0 the size is 16x16 (some are
by accident too small - not resized but taken directly fron skin_grey of 
Llasse Norby).

Other in the module bar:

Dummy icon 'ExtDevEval' is 15x12
kb_cleanfiles 18x18
mod/help/cshmanual/ext_icon 16x14

scaling all into the size 16x16 with CSS works relative well if height 
and width is the same but if not, the result is more or less bad.
At least recommendation to use module icons, which has the same height 
and width would be needed.
