Come on, there are already so many buttons in the backend, that two 
additional ones don't matter :)

It's not about moving pages or page elements, it's about rearranging 
items in a list box.
The workaround that Peter postet is not very intuitive. If you don't 
know the shift/ctrl tricks, it is quite some work to keep the order of 
the list.
Soooo... I'd vote for having at least two buttons ('one step up', 'one 
step down'). If you want, you can add two additional buttons ('to top' 
[which is already used] and 'to bottom'). That would mean three 
additional buttons in total (or 42px extra) which should be ok in most 
situations. For smaller fields that don't provide the necessary space 
(like the 'Startingpoint' field), you can leave it as is, since they are 
not intended for many list items anyway.
