I'd use both, but the FE user is set by default if present.

Yes. I think it can be really simple.

If a hook is registered but not used by any extension anymore, it will be
deleted in the core.

I can do that in the first run.
One more thought: Requested hooks could be stored in the same table but
should be disabled by default because they're not really in the core yet (I
think it's good good enough if that can be changed in the BE afterwards).
