I'm afraid we need more details in order to help you.. For instance: 
What version of Flow are you using? And...

What kind of refactorings did you do? Did you also modify framework code?
Can't you revert the refactorings to narrow down the issue?

Last but not least did you check the logs under Data/Logs and 
Data/Logs/Exceptions for more information?
