You need to create a schema migration, using doctrine:migrationgenerate
as described in
http://flow.typo3.org/documentation/guide/partiii/persistence.html#creating-migrations

With the update to Flow 2.0 you need to be careful, so check the
migration. Also, it is highly recommended to use the generic FK
constraint update technique I developed, see the Version20120930203452
migration in TYPO3.Flow.
