Uhm, hier kun je gewoon vragenvelden voor gebruiken :-)

Als dit niet gekoppeld hoeft te zijn aan de radiobuttons of checkboxes, 
maak er dan een gewoon inputveld van als aparte vraag. Eventueel kun je 
hier nog een extra CSS class aanhangen zodat het lijkt dat hij bij de 
vorige vraag hoort. Zoals je zelf al aangeeft, not part of the choice.
