Problem:
Editors are confused by the two choises of "text w. image below" and 
"Text w. image to the right" choises they get, when they want to insert 
a CE. Many logically conclude, that there only exist these two options.

Solution:
Attached patch (which was provied by Steffen some time ago) replaces the 
two options by with one choice (similar to the core's wizards).

Notes:
The patch worked fine for me - also local defVals are kept as supposed. 
Xavier also gave a "+1" within the issue-description. Any concerns?
