[TYPO3-dam-devel] RFC #18298: dam_ttcontent: Remove old and deprecated code

Hi there

This is an old-style patch request for dam_ttcontent.

Problem:
dam_ttcontent 1.2 requires TYPO3 4.5+. Therefore some old version 
switches can be removed.

A call to t3lib_div::intInRange needs to be changed to the wrapper 
method in class.tx_dam.php because these method is deprecated in TYPO3 4.6.

Also there are some remains of mmforeign that need to be removed. The 
method "access" is removed because there is no call to this function.

Needs https://review.typo3.org/#change,5483 applied to work.
