No, the image button is not enabled by the "Advanced" and "Typical" 
default configs.

It may appear however when it is enabled in Page TSConfig by other 
extensions or by the TCA settings of a particular table and field. This 
is why I recommended this change: http://bugs.typo3.org/view.php?id=2023
