The message says it all: The FORMidable extension uses some functions
that are deprecated (starting with PHP 5.3).

This has been fixed in the SVN 1.1 branch of FORMidable, but not in the
TER version yet. I've asked the people at Ameos to release a new TER
version, but nothing has happened there yet.

You could either:

1. disable PHP warnings in php.ini (I don't remember if this is also
possible in the install tool)

2. try whether the branch 1 version at <http://formidable.typo3.ug/>
solves the problem for you

3. use the 1.1 branch version from SVN
