A few random guesses:

- Please check that you have included the extension's static template in
your TS setup and *not* just copied the contents of setup.txt.

- Please check that the login box is not on the same page as the FE
editor or the image upload.

- Please check that there really is only one instance of the form on the
page. (You cannot have both the FE editor and the image upload on the
same page - they absolutely must be on separate pages.)
